Bridging the Gaps in HCBS Case Management

Staffing shortages, inadequate quality monitoring and insufficient improvement strategies are just some of the barriers contributing to gaps in care for those in need of long-term services and supports, specifically home- and community-based services (HCBS). These factors certainly presented challenges long before the COVID-19 healthcare crisis, but in the shadow of a global pandemic, their impact on some of the most vulnerable members of our communities hits even harder.  

Because of these ongoing challenges, aging and disabled populations may struggle to live and thrive at home and in their communities because they lack the proper care and services promised by state waiver programs. And we just can’t let that keep happening.

FEI’s Blue Compass case management for LTSS solution helps state and local agencies facilitate the delivery of health and human services to eligible program members. It is designed to address common requirements for Medicaid program management, while also meeting the unique and complex needs of each of the agencies and organizations we serve. The platform is also designed to address certification requirements from the Centers for Medicare & Medicaid Services (CMS) to provide our customers an opportunity for enhanced federal funding for their case management technology solutions.

With Blue Compass, agency administrators and support coordinators alike benefit from automated workflows in the management of person-centered plans of care, including everything from member intake and assessment to claims processing. The system automation can be programmed to alert staff members of tasks needing completion and/or notify stakeholders when action is required.

The platform also helps our customers effectively manage resources. Custom dashboards and reporting capabilities make it easy for agencies to pull data in real time, build custom reports, present that data in an easy-to-understand format and analyze program outcomes to inform resource allocation decisions.
